Why Twin Type Matters More Than You Might Think
When most people hear “twins,” they think identical or fraternal. But from a medical standpoint, what really matters during pregnancy is how the babies share (or don’t share) key structures inside the womb. Specifically, doctors look at two things: the placenta (called the chorion) and the amniotic sac (called the amnion).
Those two words give us the terminology you’ll hear a lot once you’re expecting twins. “Mono” means one (shared), and “di” means two (separate). Mix and match those prefixes with chorion and amnion, and you get the three main types of twin pregnancies.
Here’s the short version before we dive deeper:
- MoMo (Monochorionic-Monoamniotic): One shared placenta, one shared amniotic sac. Highest risk.
- MoDi (Monochorionic-Diamniotic): One shared placenta, two separate amniotic sacs. Moderate risk.

Your OB will typically determine twin type through an ultrasound during the first trimester, ideally between 10 and 14 weeks of pregnancy. The earlier, the better, because it shapes everything about how your pregnancy will be monitored from that point forward.
MoDi Twins (Monochorionic-Diamniotic): One Placenta, Two Sacs
This is the type my wife and I experienced with our girls, so I’ll give this one a little extra detail from personal experience.

MoDi twins are always identical. They come from a single fertilized egg that split somewhere between 4 and 8 days after fertilization. The split happened early enough for each baby to develop her own amniotic sac, but not early enough for separate placentas to form. So both babies share one placenta and its blood supply, but each lives in her own little fluid-filled bubble.
Our girls’ MoDi pregnancy went really smoothly overall. My wife had some light bed rest toward the end, and our daughters were delivered healthy at 36 weeks. Honestly, considering what a MoDi pregnancy can involve, we felt really fortunate.
That said, here’s what every dad expecting MoDi twins needs to understand: the shared placenta is the central issue that requires close attention.
The Main Risk: Twin-to-Twin Transfusion Syndrome (TTTS)
Because MoDi twins share a placenta, they also share blood vessels running across its surface. Most of the time, blood flow between the twins stays fairly balanced. But in some cases, it doesn’t, and one twin ends up receiving more blood and nutrients while the other receives less. That’s Twin-to-Twin Transfusion Syndrome, or TTTS.
According to the Cleveland Clinic, TTTS affects roughly 10 to 20% of monochorionic (shared placenta) twin pregnancies. Without treatment, the outcomes can be serious. With proper monitoring and early intervention, the vast majority of pregnancies can be managed successfully.
The good news is that TTTS is detectable through ultrasound by measuring amniotic fluid levels in each sac. That’s why MoDi pregnancies involve more frequent ultrasounds than a typical singleton or fraternal twin pregnancy.

What Monitoring Looks Like for MoDi Pregnancies
If your partner is carrying MoDi twins, expect the prenatal appointments to ramp up significantly in the second trimester. Both ACOG (American College of Obstetricians and Gynecologists) and SMFM (Society for Maternal-Fetal Medicine) recommend ultrasound monitoring every two weeks starting around 16 weeks to watch for any signs of TTTS or unequal growth.
Your doctor will likely refer you to a Maternal-Fetal Medicine (MFM) specialist, sometimes called a perinatologist or high-risk OB. That’s not a reason to panic. It’s just the standard of care for this type of pregnancy, and having that specialist in your corner is genuinely reassuring.
MoDi twin pregnancies are typically delivered around 36 weeks because the shared placenta can begin to deteriorate toward the end of pregnancy. That earlier delivery timing is standard and protective, not a sign that something has gone wrong.

DiDi Twins (Dichorionic-Diamniotic): Two of Everything
DiDi twins are the most common type, making up roughly 70% of all twin pregnancies. Each baby has her own placenta and her own amniotic sac. No shared resources at all.

DiDi twins can be either identical or fraternal. When a fertilized egg splits very early (within the first 3 days), the babies develop completely separate placentas and sacs, making them DiDi identical twins. But the majority of DiDi twins are fraternal, meaning they came from two separate eggs fertilized by two separate sperm.
From a pregnancy risk standpoint, DiDi is the jackpot of twin configurations. Because the babies aren’t sharing a placenta, there’s no risk of TTTS. The biggest concern with DiDi pregnancies is preterm labor, which is common in all twin pregnancies regardless of type.
Prenatal Monitoring for DiDi Pregnancies
DiDi twin pregnancies are still higher risk than a singleton pregnancy, but the monitoring schedule is less intensive than MoDi or MoMo. Typical care includes:
- Ultrasounds every 4 weeks through most of the pregnancy
- More frequent appointments starting around 28 to 32 weeks
- Weekly appointments approaching delivery
- Planned delivery typically around 38 weeks (most doctors won’t let DiDi twins go to 40 weeks)
You’ll likely still be referred to an MFM specialist for at least some consultations, but many DiDi pregnancies are managed by a regular OB throughout.
Are DiDi Twins Always Fraternal?
Not always, but usually. About 80% of DiDi twins are fraternal. The remaining 20% are identical twins whose egg split very early on. Without genetic testing (like a DNA test after birth), you actually can’t tell from the placenta situation alone whether DiDi twins are identical or fraternal. Your kids might think they’re just fraternal twins and find out years later they’re actually identical. It happens more than people realize.
MoMo Twins (Monochorionic-Monoamniotic): The Rarest and Highest-Risk Type
MoMo twins are extraordinary in every sense of the word. They share both a single placenta and a single amniotic sac, essentially floating together in the same space throughout the pregnancy.
MoMo twins are always identical, and they’re extremely rare. According to Healthline, MoMo twins represent approximately 1% of identical twins and less than 0.1% of all pregnancies. Some estimates put the occurrence even lower, at around 1 in 10,000 pregnancies. If your wife is carrying MoMo twins, you are in truly rare company.
The reason MoMo twins form is that the fertilized egg splits very late, typically after the 9th day following fertilization. By that point, the amniotic sac has already formed and cannot divide, so both babies develop inside it together.
The Key Risks in a MoMo Pregnancy
Because both babies live in the same sac with two umbilical cords moving freely, the biggest physical risk is cord entanglement. As the babies move around, their cords can twist or compress, which can restrict blood and oxygen flow. This risk is difficult to predict and can change rapidly, which is why MoMo pregnancies require an entirely different level of monitoring.
MoMo twins are also at risk for TTTS (just like MoDi twins) because of the shared placenta. And because of the combination of risks, fetal loss rates for MoMo pregnancies are higher than for any other twin type without aggressive monitoring and intervention.
The good news is that with modern prenatal care and intensive monitoring, outcomes have improved significantly. With proper management, survival rates can reach 80 to 95%, according to research published in peer-reviewed obstetric literature.
What to Expect During a MoMo Pregnancy
A MoMo pregnancy requires intensive monitoring. Here’s what that typically looks like:
- Early referral to a Maternal-Fetal Medicine specialist is essential and will happen right away.
- Frequent ultrasounds (sometimes multiple times per week) to check fetal heart rates and cord position.
- Inpatient hospitalization is often recommended starting around 24 to 28 weeks, so the babies can be monitored continuously around the clock.
- Planned delivery via cesarean section typically around 32 to 34 weeks, before the risks of cord complications outweigh the risks of prematurity.
If you’re facing a MoMo pregnancy, it will ask a lot of your family, your schedule, and your stress tolerance. It’s not easy. But the medical community has gotten very good at managing these pregnancies, and many families come through it with healthy babies.
How Do Doctors Determine Twin Type?
Twin type is determined by ultrasound, and timing matters a lot. The earlier in the first trimester the scan is done, the easier it is to distinguish between the types.
Between 10 and 14 weeks, doctors look for a specific tissue marker between the twins. In DiDi pregnancies, there’s a thick membrane between the babies (called the “twin peak” or lambda sign). In MoDi pregnancies, the membrane is thinner (the “T-sign”). In MoMo pregnancies, there’s no membrane at all.
As pregnancy progresses, it can become harder to determine chorionicity (how many placentas), so that first trimester ultrasound is genuinely important. Make sure your doctor confirms twin type and documents it clearly. It should be verified at each subsequent scan as well.

What About Fraternal Twins? Where Do They Fit In?
Worth a quick note here because it trips people up. Fraternal twins (also called dizygotic twins) almost always have two separate placentas and two separate amniotic sacs, making them DiDi by default. They come from two different eggs fertilized by two different sperm, so they’re essentially just siblings who shared a womb at the same time. They can be the same sex or different sexes, and they’re no more genetically similar than any other pair of siblings.
Identical twins (monozygotic) are always the same sex and share nearly all of their genetic material. Whether they end up as MoMo, MoDi, or DiDi identical twins depends entirely on when that single fertilized egg decided to split.
You’ll Have a Lot of Appointments. That’s a Good Thing.
One of the things I noticed during our MoDi pregnancy was just how many ultrasounds and checkups were on the calendar. At first it felt like a lot. Over time, I came to really appreciate each one. Every appointment where both babies looked good, both heartbeats were strong, and fluid levels were in range was genuinely reassuring.
Whatever type of twins your partner is carrying, lean into those appointments. Ask questions. Get to know your MFM specialist if you have one. The extra monitoring isn’t a sign that things are going wrong. It’s the system working exactly as it should to keep your babies safe.
